with complex heart diseases and alternative approaches: a) implantation of 1 to 3 subcutaneous electrodes as the high-energy electrode or the transvenous defibrillation electrode itself in a subcutaneous position; b) transatrial approach with implantation of the defibrillation electrode directly through the right atrium; and c) implantation of the defibrillation electrode in the pericardial sac. Given the small number of patients and the limited followup, it is hard to predict the complications associated with these procedures in the medium term. Stephenson et al proposed annual monitoring of the defibrillation thresholds, which can become fundamentally elevated with subcutaneous electrodes. Cannon et al recommend performing an annual echocardiography when there are electrodes in the pericardial sac to detect the possibility of progressive strangulation of the myocardium. This technique is considered preferable in patients with larger body surface areas, as was the case in our patient. Due to the increase in the population with congenital heart disease reaching adulthood, we will need to use this and other approaches increasingly often.

Percutaneous treatment of a dysfunctional aortic bioprosthesis with the CoreValve(®) prosthesis.
Recently, percutaneous aortic valve replacement has emerged as a therapeutic option for patients with severe symptomatic aortic stenosis and a high surgical risk. We report our initial experience in four patients with percutaneous implantation of a CoreValve aortic prosthesis to treat aortic bioprosthesis dysfunction involving aortic stenosis or regurgitation. In-hospital and medium-term outcom...
متن کاملPercutaneous aortic valve implantation retrograde from the femoral artery.
BACKGROUND Percutaneous aortic valve implantation by an antegrade transvenous approach has been described but is problematic. Retrograde prosthetic aortic valve implantation via the femoral artery has potential advantages. Percutaneous prosthetic aortic valve implantation via the femoral arterial approach is described and the initial experience reported. METHODS AND RESULTS The valve prosthes...
متن کاملAcquired Infrarenal Abdominal Aortic Coarctation: Treatment with Percutaneous Self Expandable Stent
Abdominal aortic coarctation is an extremely rare vascular pathology. Its etiology can be congenital or acquired. Here we present a case of acquired infrarenal abdominal coarctation in a 66-year-old woman who complained of low back and legs pain. She had no signs of resting lower limb ischemia, with diminished distal pulses and normal blood pressure in upper and lower extremities. Magnetic reso...
متن کاملChanges in atrioventricular conduction and predictors of pacemaker need after percutaneous implantation of the CoreValve®. Aortic valve prosthesis.
INTRODUCTION AND OBJECTIVES Although changes in atrioventricular conduction frequently occur after percutaneous implantation of an aortic valve prosthesis, little is known about the mechanisms involved or how these changes progress. We investigated ECG abnormalities and predictors of pacemaker need after percutaneous implantation of the CoreValve® aortic valve prosthesis. METHODS Between Apri...
متن کاملPercutaneous Implantation of the CoreValve Self-Expanding Valve Prosthesis in High-Risk Patients With Aortic Valve Disease The Siegburg First-in-Man Study
Background—The morbidity and mortality of surgical aortic valve replacement are increased in elderly patients with multiple high-risk comorbid conditions. Therefore, a prospective, single-center, nonrandomized study was performed in high-risk patients with aortic valve disease to evaluate the feasibility and safety of percutaneous implantation of a novel self-expanding aortic valve bioprosthesi...
متن کاملAN INNOVATIVE SURGIC AL TECHNIQUE FOR CONTINUOUS SUTURING OF PROSTHESIS IN MITR AL VALVE REPL A CEMENT
Mitral valve replacement (MVR) is usually performed in two methods of running and interrupted suturing. In running method, the suturing of the valve is usually with some traction on the annulus and also there is a risk of aortic valve injury. This study was conducted to evaluate a new technique for suturing to avoid these complications and decrease the time of cardiac arrest. One hundred an...
